Paperwork Requests – Cerebral

ℹ️ Cerebral recommends clinicians have a 6 month established relationship with a client [defined by the date of the first session] before completing certain paperwork requests.

This pertains to minimum eligibility and does NOT guarantee the client will receive any requested paperwork after the 6 month window has passed.
Exceptions may be made on a case-by-case basis if deemed clinically appropriate.

🗒️ Clinicians can deny a paperwork request for any reason.

Clinicians have the right to request for our legal team review any paperwork.
Clinical leadership cannot assist with paperwork requests.

⚠️ Therapist associates [TAs] cannot complete paperwork requests.

If a client assigned to a TA needs to submit a paperwork request:
- Inform them that TAs can't complete paperwork.
- Ask if they want to be reassigned to a fully licensed therapist who can complete paperwork.

🗒️ Account status considerations:

We cannot complete paperwork requests for cancelled or paused clients.
If a client is scheduled for cancellation and has an in-progress paperwork request, we can continue working towards completion.

⚠️ Paperwork requests that require clinician completion must be submitted to the prescriber for review for clients on medication-inclusive plans.

Emotional Support Animal [ESA]

Document recommending that the client could benefit from the use of an assistance animal to provide emotional support.

Appointment confirmation

Document confirming that the client had a session at a specific date/time.

ℹ️Can be completed without clinician involvement unless a clinician signature was requested.

Letter of care

Document confirming that the client has been under our care for the management of a specific diagnosis during the provided timeframe.

ℹ️Can be completed without clinician involvement unless a clinician signature was requested.

🗒️ If the client requires additional details of their care such as medications, we can provide them with their medical record. Otherwise, we should provide a letter of care.

Summary of medications

Document providing a summary of the client’s treatment plan.

Letter w/accommodations

Document confirming that the client has been under our care for the management of a specific diagnosis and as a result of that is requesting specific accommodations.

Letter of medical necessity

Document typically provided to an insurance provider as proof of payment for services.

⚠️ Required if requested through insurance.

FMLA/short term disability

Document that grants eligible individuals up to 12 weeks of unpaid leave [with benefits] per year from their job for certain family and medical reasons.

⚠️ Cerebral cannot accommodate the following paperwork types:

Service animals
Court-Mandated Mental Health Evaluations
DWI/DUI/Substance Use Assessments

ℹ️ Cerebral requires 15 business days to complete any paperwork request.

Clinicians have 10 business days to complete their part of the paperwork request.
The 10 business day window begins after the clinician has agreed to complete the paperwork request.

ℹ️ When we receive FMLA/disability/legal paperwork for a client that does not have a 6 month established relationship with their clinician, we will deny the request.
