HIPAA Authorization requests via physical mail

If a previous client emails us wanting to send a HIPAA Authorization via physical mail:

• Inform them that this is possible, however it will delay the process.
• Provide them our mailing address: Cerebral Inc., 2093 Philadelphia Pike, #9898, Claymont, DE 19703. 

Personal Representatives

• A client may appoint a personal representative to request & receive documents & records on their behalf. 
• To do so, the personal representative must provide documentation proving they have authorized access.
• Clients can email privacy@cerebral.com for guidance.
