Complex Case Managers [CCMs]

ℹ️ Effective 10/3/2022, the Complex Case Manager program was implemented as a new care offering for higher acuity clients who may need additional support navigating their care inside or outside of Cerebral.

Support team crisis response protocol

⚠️ If you are questioning whether something might be a concern for client safety, treat it like it is one and initiate crisis response protocol.

If a client says something concerning like they are in a mental health emergency or medical emergency, initiate safety protocol.
