In order to best preserve your privacy and protected health information, Cerebral asks that you upload a picture of your face and valid government-issued identification at the time of sign-up. Your identification and a picture of your face will be used to verify your identity during your video visits and when sending medication to your pharmacy. Without a valid government identification and picture of yourself, your Care Team will be unable to conduct visits, and will subsequently be unable to prescribe medication.
Further, Cerebral requires valid government ID to diminish the likelihood of identity theft. You will need a valid, government-issued identification at every video visit and must present it at the beginning of your visit.
