To request these documents, please send us a message from your client account, and our Support team will promptly forward your request to your dedicated Care Team.
Your Care Team, comprised of your Prescriber or Therapist, will carefully review your request to determine if it is clinically appropriate to fulfill. In certain cases, your Care Team may feel that their understanding of your background is limited and they are unable to provide the requested documents. However, we are always happy to forward the request to them for their input and consideration.
If your Prescriber or Therapist approves the paperwork request, please note that turnaround times can be up to 15 business days to ensure thorough and accurate completion.
In situations where your Prescriber or Therapist is unable to complete the requested paperwork, we can provide you with a copy of your medical records along with a letter of care, which confirms that you are utilizing Cerebral's services.
